Individual Person Centred Transition Pathway

Young people should always be central to their transitions planning.  Information about what is important to them now and in the future needs to be gathered with the young person, in the most appropriate format to that young person.  This needs to be done in a timely way, so that the information gathered can be shared with commissioners and planners.
